PHYSICAL EXAMINATION

 School nurses will strongly recommend annual well child health history, physical and dental examinations by the child’s private physician according to guidelines of the American Academy of Pediatrics. Forms to be completed by a physician or health care provider will be distributed annually to update health information on each child.  Students wishing to participate in any school sponsored athletic activities are required to have an annual physical conducted after June 15th and completed prior to participation in practices or athletic contests.
