SCHOOL HEALTH RECORDS

A Permanent School Health Record will be maintained on every student in the school district. The record shall contain immunization history, screenings’ results, and any other pertinent health information. These records are confidential and will be shared with only those persons designated by the Family Education Rights and Privacy Act (FERPA). A student record will be transferred to another school district/system with parental permission.

School logs are to be kept listing any child who is sent home ill or is seen by the school nurse.
